Forms and Certificates

All 4-H Youth participating in the 2024 Cattaraugus County Fair must submit all the proper documents by June 15th, 2024

• Entries for the Cattaraugus County Fair 4-H Horse Show
• NYS Horse Certificate
• A Riding/Driving Evaluation Form for every horse/rider combination
  • Cloverbuds must submit an "On-the-Ground Skills" Evaluation Form
• Proof of a current Negative Coggins Test (must be completed by your veterinarian)
• A current Rabies Certificate (must be completed by your veterinarian)
