2025-26 Youth Enrollment Form

By clicking the link above, you can access the Youth Enrollment Form for the 2025-26 4-H year. To enroll, complete the form and return to the 4-H office. There is an enrollment fee of $10/child or $25/family. Make checks payable to CCE of Cattaraugus County. Please contact the 4-H office with any questions.
